Immunizations
It is state law that all students have the following immunizations on file in the school that they attend.
DPT/DT/TD - Five doses required, unless 4th dose was given after 4th birthday, then one dose every ten years.
POLIO - Four doses, unless 3rd dose was given after 4th birthday.
MMR - Two doses, 1st dose on or after first birthday; second dose
by age 5 or entry to Kindergarten.
HEPATITIS B - Three doses required for the following grades in the following school years:
2006-2007 K-12
VARICELLA - (Chickenpox) - One dose of vaccine on or after 1st birthday or proof of disease for the following grades:
2006-2007 K-12
(two doses required if vaccine given at 13 years of age or older)
